PAQ, Inc. offers medical coverage for our employee owners and their dependents, which include comprehensive coverage for office visits, hospitalization expenses, diagnostic services, routine exams, screenings, and prescription coverage. LiveHealth Online

Life Insurance/Voluntary Insurance Policies
PAQ offers a free $50,000 Group Life Insurance and Long-Term Disability policy to regular employees working 30+ hours per week. Employees can purchase additional Accident, Critical Illness, Hospital and Voluntary Life Insurance policies for themselves, spouse and children through Mutual of Omaha. Open enrollment for Voluntary Life is during October and effective January 1 and is subject to completion of an Evidence of Insurability form. For more information click on the links below:

LegalShield/IDShield
LegalShield/IDShield provides you access to a network of lawyers throughout the US and Canada to help in any personal legal matter, no matter how trivial or traumatic, at a sufficiently reduced rate. They also provide identify theft protection and restoration through IDShield. Coverage is at the employee owner’s expense. Enrollment begins in October for an effective date of January 1. PAQ, Inc. sponsors a self-funded medical plan. BRMS TPA provides PAQ with a Uniform Resource Locator (URL or website) where Machine Readable Files (MRFs) for the plan will be posted publicly and updated monthly as of 7/1/22. Please note that these files are not consumer friendly but meet the approved format. The consumer-friendly price transparency tools won’t be available until 2023 and 2024.
